THE HOLISTIC THERAPY ACADEMY ENROLLMENT CONTRACT

 

 

The Holistic Therapy Academy agrees to enroll the above named Participant and to provide program, educational, and other services.

 

In consideration of the acceptance of the Enrollment Contract by the Holistic Therapy Academy, the undersigned agrees to pay the required fees as specified below:

 

 ALL PAYMENTS ARE NON-REFUNDABLE

 

  • Tuition for the  school year is $6,504. Select one of the following options:

 

Option #1 - Payment Plan 12 Payments of $542.00

 

I (we) understand that my (our) obligation to pay tuition and fees for the full enrollment year is unconditional

 

Late Fee Assessed:

  If a payment fails and fails again after 7 days there will be a $100 late fee charge added to that months tuition.   Please note that the portal automatically removes access to the course for failed payments.   If you need to make any changes to your card or payment date please email [email protected]

 

EXTENSION POLICY

We review extension requests on a case by case basis and reserve the right to decline or accept requests. We give 30 days past the end date for students to get all final projects and assignments completed and turned in after that if there are extenuating circumstances we will assess on a case by case basis to determine if we can grant an extension.

 

The Holistic Therapy Academy reserves the right to legal action for collection of school tuition and fees. Return check fees and costs of collection will be added to your account.

 

The Holistic Therapy Academy reserves the right to suspend or expel a Participant of the Academy for any reason or cause. In event of such the Participant is still required to fulfill all financial obligations.



I(we) understand that in signing this Enrollment Contract for the coming semester, I(we) agree to accept the rules and regulations of the Academy.

 

 

All materials that they received from the Academy (manuals, videos, practices) are for personal use only and they may not share any of the    (lectures, manuals, or any of the materials) with colleagues. Login information must be kept confidential. Any violation of the aforementioned will result in immediate expulsion from the Academy.

 

Participants acknowledge that the lessons and practices of The Holistic Therapist Academy are the intellectual property of LeNaya Smith Crawford LLC and will remain the exclusive property of LeNaya Smith Crawford LLC and will only be used by the Participant in their own private practice and/or business creation. The Participant will not use practices or lessons from The Holistic Therapist Academy said practices or lessons to copy or duplicate The Holistic Therapist Academy. The Participant will not use said practices or lessons that might be directly or indirectly detrimental to the Disclosing Parties or any associated affiliates or subsidiaries.

 

The undersigned agrees to release and hold harmless the school, its agents and employees from all claims, damages or other liabilities for injuries which are not the result of gross negligence by this school, its agents or employees. The undersigned also agrees to indemnify the school for damages by participant.

 

This contract shall be interpreted in accordance with the laws of the State of Georgia.
